Consent & Story Use Agreement - PLEASE READ THROUGH ENTIRELY

Thank you for trusting us with your story. Before submitting, please review the following and indicate your preferences for how your story may be used.

By checking the box below, you give Crime Victim Center (CVC) permission to use your submitted story (written, audio, or video) for awareness and outreach purposes. This may include, but is not limited to:

CVC’s website

Social media platforms

Email communications to donors and supporters

Printed materials, such as newsletters or annual reports

Event presentations (in-person or virtual)

You may choose to remain anonymous, or allow your first name and/or image/audio to be used. You can also request that your story only be used internally or not shared at all.

While we take steps to protect your privacy, please be aware that uploading personal stories may carry emotional or digital risks. If you are using a shared or public device, we recommend logging out of your account after submission.
